About The Position

Catholic Charities Maine is seeking a compassionate and highly skilled Clinical Care Coordinator to join our Adult Opioid Health Home (OHH) team. In this vital role, you will support adults living with opioid use disorder and co-occurring mental health challenges by coordinating integrated behavioral and physical health services that empower individuals to reach their highest level of wellness and recovery. The Adult Opioid Health Home (OHH) Clinical Care Coordinator shall be a trained, supervised professional who will work as an integral part of the OHH Team to coordinate and monitor the delivery of effective, integrated behavioral and physical health services to enrolled OHH members. She/he will provide clinical support to include individual and group sessions focused in the areas of treatment planning, relapse prevention, safe approaches to harm reduction, and motivation along with coordination and management of the overall integration of the member’s emotional and physical health supports and services toward assisting them in achieving their optimal health/wellness goals.

Requirements

  • Bachelor’s degree in Psychology, Human Services, or related field.
  • Current MHRT/C certification and CADC required.
  • Minimum 2 years working with adults experiencing mental illness and addiction in a clinical or community-based setting.
  • Strong knowledge of addiction, mental illness, and community resources.
  • Proficiency in electronic health records.

Responsibilities

  • Coordinate and monitor the delivery of effective, integrated behavioral and physical health services to enrolled OHH members.
  • Provide clinical support including individual and group sessions focused on treatment planning, relapse prevention, and harm reduction.
  • Assist members in achieving their optimal health and wellness goals.
