About The Position

Do you have the career opportunities as an Adult Oncology Nurse Navigator you want with your current employer? We have an exciting opportunity for you to join HCA Florida Trinity Hospital which is part of the nations leading provider of healthcare services, HCA Healthcare. This navigation position is based out of HCA FL Trinity Hospital Area and focuses on Complex GI and Colorectal oncology patients. Schedule: This is an exempt position that will work Monday through Friday 8 am to 5 pm.

Requirements

  • Three to five years oncology nursing experience.
  • Experience in program development preferred.
  • Experience as a nurse navigator and/or patient educator in oncology preferred.
  • Experience in leadership and customer service - preferred.
  • Associate's Degree in Nursing required.
  • Current licensure as a Registered Nurse (RN).
  • Basic Cardiac Life Support (BCLS) certification.
  • Oncology Certified Nurse (OCN) achieved or in progress - preferred.

Nice To Haves

  • Bachelor's Degree in Nursing preferred.

Responsibilities

  • Serve as a liaison between the patient and family with the multidisciplinary healthcare providers to ensure effective communication and collaboration.
  • Collaborate with local physician liaisons and leadership to conduct internal and external outreach and marketing when appropriate.
  • Serve as a patient advocate from diagnosis to end of active treatment.
  • Assess patients' medical, social, and psychosocial needs/barriers to care and link them to appropriate resources.
  • Provide appropriate teaching, outreach, and support to patients and families and ensure the patient is empowered to manage their own healthcare needs.
  • Conduct follow-up conversations with all patients and communicate any concerns, changes, or social needs to the appropriate physician or other care providers.
