About The Position

Sunrise Services, Inc. is seeking caregivers for our Adult Family Home! About the Program: We are looking for Caregivers (HCA or CNA highly preferred) who can provide whatever support and assistance is needed by AFH residents in their daily lives and documents services given. Candidates need to be caring, patient, and reliable and must possess good communication skills and have knowledge of de-escalation techniques. Support and assistance is provided in areas including, but not limited to, the following: Health and safety Power and choice Positive recognition by self and others Integration into the physical and social life of the community Developing relationships Promoting competence and self-reliance Maintain participant records CARE assessments, Negotiated Care Plan (NCP) and addendums, Positive Behavior Support Plan (PBSP) and/or Crisis Plans (if applicable), Safety Plans (if applicable): Be able to describe the purpose of and have full and complete knowledge of the contents of the above plans. Follow these plans exactly as written Assist and support residents as directed in CARE assessments, NCP's, and Safety Plans. Support the participant in accordance with the guidelines outlined in the PBSP and/or Crisis plan. Give timely prompt feedback to the RM on issues related to these plans. Shift: Full-time: Sunday - Thursday 11pm - 7am

Requirements

  • Must have High school diploma or GED and be at least 18 years of age.
  • First aid and CPR, BBP and food handler's card is required within 30 days of hire.
  • Excellent communication (oral and written), listening, organization, documentation and problem-solving skills required.
  • Creativity, flexibility, dependability, positive attitude, and sensitivity to diverse populations.
  • Ability to organize work, time, and priorities to meet resident needs.
  • Ability to role model, maintain professional boundaries, and establish and maintain appropriate working relationships with others (both within and outside the company).
  • Ability to sit or stand for extended periods of time.
  • Ability to operate or use basic home appliances such as vacuum, microwave, dishwasher, etc. and household tools such as mop and broom.
  • Ability to use a calculator to perform simple math functions required.
  • Ability to lift approximately 20-30 pounds and physically assist unsteady clients

Nice To Haves

  • Home Care Aide (HCA) or Nursing Assistant Certification (NAC) preferred.
  • Prefer at least one year of experience working with people with disabilities or experience working in a similar industry.

Responsibilities

  • Provide support and assistance to AFH residents in their daily lives
