Position: Adult Family Care Case Manager Location: Wakefield, MA Pay rate: $49,920 exempt annual salary Hours: Full-time, 40 hours per week – Hours worked at the Communitas office in Wakefield and in the community attending home visits (Some work from home hours can be accommodated upon completion of 90-day probationary period) Benefits of Working for us! This position requires the successful candidate to complete home visits in various locations (this includes Essex, Suffolk, and Middlesex county – locations up to 30 min drive from office) at various times based on family’s schedules. This often includes some early evening hours after 4pm.
