About The Position

As an Adult Day Care Professional, you will provide direct support, case management, and program coordination for adults with disabilities in a structured day services setting. You’ll help participants build independence, develop skills, and engage in their community while ensuring a safe, supportive, and person-centered environment.

Requirements

• Must be 18 years or older
• High school diploma or equivalent
• Experience in human services or working with individuals with disabilities
• Strong communication and organizational skills
• Basic computer skills (Microsoft Office)
• Valid driver’s license and reliable transportation
• Ability to work independently and as part of a team

Nice To Haves

• Associate’s or Bachelor’s degree in a related field
• Experience in adult day care or similar programs
• Case management or service coordination experience

Responsibilities

• Provide hands-on support following individual service and behavior plans
• Lead and assist with group and one-on-one activities
• Support personal care, meals, and daily living skills
• Encourage community outings and social engagement
• Monitor health and safety and respond to concerns
• Manage a caseload of approximately 8–10 participants
• Complete documentation including case notes, progress updates, and reports
• Participate in care planning and team communication
• Advocate for participant goals and service alignment
• Transport participants as needed (including wheelchair-accessible vehicles)
• Assist with route coordination and daily attendance needs
• Communicate schedule or coverage concerns
• Follow agency policies and regulatory standards
• Attend meetings and required trainings
• Maintain clear communication with supervisors and team members
