About The Position

Full-time position for Mental Health Case Manager to assist adult clients living with severe mental illness in Cincinnati/Hamilton County. Position includes working with clients who participate in specialized programs such as LGBTQ+ and older adult populations. Provide community-based case management services such as but not limited to, symptom monitoring, treatment/service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral/advocacy to community support systems and support of client’s self-determination. Work in a supportive team environment as well as collaboration with a multi-disciplinary clinical treatment team. Meet service delivery expectations.

Requirements

  • High School Diploma or Equivalent
  • Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability
  • Strong interpersonal skills and ability to provide support in person and by telehealth
  • Must be able to lift/push/pull up to 20 pounds to support demands within office environment

Nice To Haves

  • bachelor’s degree is Preferred
  • Case Management experience is not required
