General Statement: Under the direction of the Program Supervisor, functions as a team member providing professional services in accordance with the person-centered plans of the person served, and coordinates the provision of supports and services for adults with mental illness, substance use, co-occurring, and/or developmental disabilities who need assistance in accessing, coordinating, and monitoring resources and services. Assists the individual and their support network to identify, select, obtain, and coordinate both paid and unpaid or natural supports to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in their Individualized Plan of Services (IPOS); performs related work as required.

  • Functions as a team member providing professional services in accordance with the person-centered plans of the person served

  • Performs related work as required
  • Bachelor’s degree in Human Services (Or a related field such as Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Counseling, Criminal Justice, Recreational Therapy, Rehabilitation Counseling, Social Work, Psychology, Education, or Sociology).
  • Must have a valid Michigan Driver’s License, current automobile insurance, and use of a personal vehicle during work hours.
  • Must be willing to transport persons served using personal or agency vehicles.
  • Provides services in office, home, and community locations.
  • Able to obtain and maintain MDHHS credentials as a Qualified Mental Health Professional and/or a Qualified Intellectual/Developmental Disability Professional (QIDP)
  • LBSW AND one (1) year of clinical experience working with persons diagnosed with mental illness, substance use disorder, and/or developmental disabilities.
  • Fluency in American Sign Language and other manual methods of communication with Deaf, deaf, and Hard of Hearing persons. Evidence of American Sign Language Proficiency Interview (ASLPI) with a minimum score of three (3) (Intermediate Level). It is required that all candidates utilizing American Sign Language while providing services MUST demonstrate fluency in American Sign Language and other manual methods of communication with Deaf, deaf, and Hard of Hearing persons by passing the American Sign Language Proficiency Interview (ASLPI) with a minimum score of three (3) (Intermediate Level).
  • MCBAP Development plan in progress or MCBAP certification
