About The Position

Adult Case Manager Mon - Fri - 8am - 5pm Full-Time GENERAL RESPONSIBILITIES: A certified case manager works to provide quality case management services in an outpatient or community setting designated by CBH. The primary function of the adult case manager is to provide goal directed supports and solution focused interventions intended to achieve identified goals or objectives as set forth in the consumer’s individualized treatment plan. CPST (Community Psychiatric Support and Treatment) is a face to face intervention with the consumer present: however, family or other collaterals may also be involved. These services are provided in both agency and community settings, such as the client’s residence. The case manager is responsible administratively to the Program Supervisor.

Requirements

  • A BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.
  • Staff members shall possess demonstrated interpersonal skills, ability to work with the mentally ill, ability to respond effectively in a wide variety of human service situations.
  • Experience working with persons diagnosed with a Severe and Persistent Mental Illness is preferred.
  • Must have a current and valid driver’s license.
  • Must successfully complete Case Management Certification as stipulated by the state of Kansas within six months of employment and maintain Case Management Certification thereafter.
