About The Position

To provide CPST, TCM, and additional rehabilitative services to SPMI adults and/or adults meeting functional criteria in Cherokee County. See definitions below. Primary Duties and Responsibilities Provide quality, medically necessary case management services to adults with a severe and persisted mental illness. Meet client engagement expectations and provide service delivery in accordance with level of care. To ensure that referrals for CPST or TCM services are acted upon and within 3 business days of referral. To ensure that clients will be assessed via the strengths/needs assessment tool. Monitor and provide timely documentation of all services provided in accordance with policy and SRMHW’s accepted standards. To attend workshops and complete training requirements as necessary for provision of CPST; TCM; and additional Rehabilitation Services, as needed. Provide mobile crisis response according to SRMHW’s policy. Complete additional duties, as assigned.

Requirements

  • BA/BS degree in human service field or be equivalently qualified through a combination of years of experience and education.
  • Possess demonstrated interpersonal skills, ability to work with person with severe and persistent mental illness, and the ability to react effectively in a variety of human service situations.
  • Prior experience in mental health or human services field.
  • Meet specifications outlined by the Division of Healthcare Policy.
  • Completion of background checks and pre-employment drug screening required.
  • Educational background and demonstrated knowledge of mental health needs.
  • Knowledge of strength based assessments.
  • Knowledge of community resources and referral agencies.
  • Skilled in establishing and maintaining effective working relationships with clients; staff; and the public.
  • Ability to write reports and document client progress.
  • Ability to organize and track program evaluation data.
  • Ability to utilize computer software efficiently.
