We are seeking a Full Time Adult case manager for our Presque Isle Outpatient Office. Position Summary The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more. You’ll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.

Requirements

  • Strong interpersonal and communication skills
  • Passion for mental health, community support, and advocacy
  • Ability to work collaboratively in a team environment
  • Flexibility and willingness to meet clients where they are (including community-based work)
  • Experience with behavioral health, case management, or human services preferred
  • The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field.
  • Bachelor’s degree preferred.
  • Must be certified at the full MHRT-C or provisional level.
  • Use of personal vehicle and active driver’s license is required.

Responsibilities

  • Build strong, therapeutic relationships with clients—including those who may be harder to engage
  • Help clients set and achieve personalized wellness goals
  • Provide skill-building and psychoeducational support (individual & group settings)
  • Coordinate care plans and connect clients to essential community resources
  • Support clients and families in making informed decisions
  • Assist with access to medical, behavioral health, and social services
  • Provide crisis intervention when needed
  • Maintain accurate documentation and meet service delivery standards
