About The Position

The Adult Case Manager position is designed to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual client needs. The role aims to enhance the quality of client management and satisfaction while promoting continuity of care and cost-effectiveness through integrated case management functions, utilization review, and discharge planning.

Requirements

• Minimum of a baccalaureate degree from an accredited university in psychology, Social Work, Health Education, or a related human services field.
• At least one (1) year of full-time or equivalent experience working with the target population who have serious mental illness/substance use.
• Skilled in community organization and familiarity with community resources.
• Completion of AHCA approved case management training or willingness to complete it within six (6) months of employment.

• Bilingual in Spanish/English (Preferred)

Responsibilities

• Conduct comprehensive client assessments to develop individualized service plans.
• Coordinate discharge planning and ensure compliance with HIPAA regulations.
• Manage patient services and maintain accurate medical records.
• Provide case management support, including intake processes and follow-up care.
• Collaborate with healthcare teams to optimize client outcomes in various settings, including inpatient and outpatient care.
