About The Position

The Department of Mental Health (DMH) is seeking an experienced Adult Case Manager to provide comprehensive support and advocacy for eligible adults in Southeastern Massachusetts. This role focuses on assessment, outreach, service coordination, and promoting recovery through person-centered engagement. The Adult Case Manager will work collaboratively with clients, service providers, and families to develop and implement individualized service plans, ensuring the highest level of respect and dignity for those served.

Requirements

  • Demonstrated skills in counseling, interviewing, engagement, motivation, and reinforcement techniques.
  • Experience with the diagnoses and symptoms of mental and emotional disorders and knowledge of available programs and services.
  • Expertise in general report writing methods.
  • Knowledge of crisis intervention principles and practices, risk mitigation, and group counseling processes.
  • Ability to prioritize and multitask effectively.

Nice To Haves

  • Knowledge of psychosocial problems related to mental/emotional, substance use, and co-occurring disorders.
  • Bilingual or multilingual fluency, particularly in Mandarin and/or Vietnamese.

Responsibilities

  • Review eligibility determination records and obtain clinically relevant information through various sources.
  • Develop Comprehensive Assessment and treatment plans (ISP), regularly reviewing and modifying them as necessary.
  • Schedule multidisciplinary team meetings to formulate treatment recommendations and maintain effective communication with service providers.
  • Conduct in-person visits and maintain regular communication with clients, service providers, legal guardians, and family members.
  • Make referrals for services when necessary and ensure updated collateral information is included in client charts.
  • Maintain both electronic and paper medical records with up-to-date information on persons served.
  • Participate in meetings and case conferences.
