About The Position

The Opportunity Alliance is seeking a Care Coordinator / Case manager to join our Behavioral Health Home team. This Adult Care Coordinator/ Case Manager position is a community case worker that provides direct service for our adult Behavioral Health Home (BHH). This position contributes to program outcomes through the provision of care coordination for BHH members supporting a recovery oriented, integrated health service that strives to improve health outcomes, cost effectiveness, and enhanced consumer engagement. Work is conducted at members’ homes, at providers’ offices, at TOA locations and in other community settings. Case Manager /Care Coordinators, participate in program initiatives, attend team meetings, supervision and work groups, coordinate service teams for each client, use information to improve health outcomes and complete documentation in a timely fashion. Work is performed, reviewed, and evaluated under the Director through staff meetings and individual supervision. This position will transport clients, attend treatment team meetings, access community resources and participate in service team meetings by using their own transportation.

Requirements

  • High school diploma or equivalent.
  • One year of relevant adult behavioral health work experience as an MHRT-1.
  • Certification: MHRT-C certification or (level A or B with a strategy to receive full certification).
  • Experience doing direct service with adults who struggle with mental health.
  • Ability to pass a criminal background check, child protective service check & sex offender check.
  • Good driving record.

Nice To Haves

  • Associate’s Degree in mental health, psychology, or a related field.
  • Case management experience with adults who have serious and persistent mental illness.
  • Experience with clients having a history of trauma, substance use, homelessness, poverty, chronic pain, and serious health issues.

Responsibilities
