About The Position

Responsible for the case management of the Adult Day Training levels 3 and 4 program participants and their rehabilitation process by providing them with individualized counseling and training services in the areas of personal and social adjustment, living skills, basic academic skills, work habits and skills, attitudes and values, work experience, self-advocacy, etc., to promote and increase their independence, personal choice, and self-sufficiency.

Requirements

  • Bachelor’s Degree from an accredited institution. Experience may be considered instead of education at the discretion of the Vice President of Mission Services.
  • Experience in using various career exploration tools and assessments.
  • 2+ years of career counseling experience in a non-profit setting preferred. 2+ years of case management experience in a non-profit setting preferred.
  • Experience in assessing aptitudes, abilities, needs, interests, readiness for employment, and barriers that hinder training and jobs for people with developmental disabilities is highly desired.
  • Strong counseling and coaching skills. Excellent interpersonal and written communication skills.
  • Previous experience in human services or special education is a plus.
  • Computer literate. Excellent organization, communication, and interpersonal skills.
  • Must have the ability to establish and maintain good working relationships with community partners.
  • Must be flexible, multi-task oriented, and able to prioritize job duties with minimal supervision, including flexibility in work hours.
  • Must always exercise discretion and confidentiality.
  • Must complete the required training and attend meetings.
  • Must have good written and oral communication skills necessary to perform job duties.
  • Must follow all safety procedures and correct safety issues, if possible.
  • Must have a valid driver’s license and have access to a reliable vehicle.
  • Able to operate a computer and MS Office software, including Word and Excel.

Responsibilities

  • Conducts the enrollment and termination process of participants in the ADT program, monitors daily attendance records, follows up on participants’ absences, and maintains and documents communication with referring sources.
  • Supervise participants, resolve and follow up on their performance and/or behavioral needs, provide individual counseling, and document interventions.
  • Provides career navigation, coaching, and stabilization resources to facilitate positive outcomes for participants seeking paid employment in integrated community settings or to advance in career opportunities.
  • Conducts assessments for training and/or employment needs; evaluates their interests, skills, and abilities, guides program activities, and plans training services with Skills Trainers, as appropriate.
  • Meets regularly with Skills Trainers to discuss participants’ progress and add input and case notes in the Daily Performance Measurement (DPM).
  • Develops and evaluates an Individualized Program Plan (IPP) for each participant based on goals from the State Support Plan and results of the assessments. Documents plan goals, progress, and develops Quarterly and Annual reports by state regulations.
  • Maintains detailed case records of participants and completes satisfaction surveys of services according to State and/or agency requirements.
  • Ensures that participant information is promptly entered into the appropriate forms, reporting system, and agency and/or State database, while maintaining confidentiality requirements.
  • Complies with the agency’s policy and procedures, the iBudget Handbook guidelines, and the Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
  • Performs other duties as assigned for the efficient functioning of the department and the well-being of the participants.
