About the position

The Adolescent Care Coordinator/LPN will be clinically competent while delivering care to patients, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. The Adolescent Care Coordinator/LPN will effectively interact with patients, significant others and other health team members while maintaining all standards of professional nursing. The position is located in Brentwood, TN and is onsite. The salary ranges from $55,000 to $65,000, dependent upon licensure. The patient population includes adolescents ages 10-18, and the working hours are Monday to Friday from 10am to 6pm, with no holidays, weekends, or on-call duties.

Responsibilities

  • Assess and monitor clients’ health status and physical care needs at admission and while clients are in the program.
  • Complete an admission nursing assessment for every client according to program standard.
  • Regularly obtain and document patients' vital signs and weights according to program standard.
  • Communicate relevant clinical and other client care information to program director, medical director and treatment team members as necessary and appropriate.
  • Monitor clients on an ongoing basis to identify health needs, problems, and/or symptoms.
  • Assist patients with personal care needs as needed or requested.
  • Coordinate and document patients care in cases of medical emergency.
  • Assist with ensuring regulatory compliance.
  • Implement and document nursing interventions that are part of the interdisciplinary treatment plan.
  • Participate in the development of a therapeutic alliance between the interdisciplinary treatment team, patients, and their families.
  • Adhere to safety and infection control guidelines.
  • Participate in the completion and maintenance of patients' medical records, including recording of physician’s orders.
  • Participate in quality improvement activities as requested.
  • Provide health education for patients, families, and community groups as needed.
  • Provide education to the community on mental illness or related health topics as assigned by the Program Director.
  • Provide families and family caregivers with education on health-related topics.
  • Conduct community meetings as requested.

Requirements

  • Graduation from a state approved school of nursing.
  • Licensed Practical Nurse or Registered Nurse in the State of practice.
  • Psychiatric Nursing experience preferred.
  • Patient and family teaching skills.
  • Teaching/learning materials development skills.
  • Problem solving skills.
  • Complaint resolution skills.
  • Ability to make presentations to outside groups.
  • Good problem solver.
  • Results oriented.
