About The Position

As the Admissions RN, you will be responsible for performing admissions at a patient's home, skilled nursing facility, or assisted livings. This role offers $78,000 - $95,930.16 annually based on experience!
Requirements
- NJ RN license required, BSN Degree preferred. CHPN or OCN preferred, BLS training
- NJ driver’s license and insurance coverage required
- Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred.
Responsibilities
- Visit patients/families, physicians/health care professionals in hospitals and other settings to facilitate admission to hospice program
- Organize discharge plan for hospitalized patients
- Document in EMR in concise and timely manner per standards
- Follow OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biologicals
- Collaborate with Referral Center staff on an on-going basis
- Inpatient evaluations.
