Requirements
- Must be at least 21 years of age.
- A valid non-restricted RN license in the state of Colorado is required.
- Must have a valid non-restricted driver’s license.
- Flexibility and availability to work a weekend schedule if needed.
Nice To Haves
- At least 1 year of hospice or home health experience preferred.
Responsibilities
- Serve as a trusted guide to patients and families, explaining hospice care and answering questions with empathy and clarity
- Conduct patient assessments, obtain consents, and initiate individualized care plans
- Collaborate closely with our interdisciplinary team to ensure a smooth, comforting transition into care
