About The Position
The Admissions Nurse (LPN) is responsible for coordinating and completing the clinical admissions process for new residents, ensuring accurate assessments, documentation, and a smooth transition into the long-term care facility.
Requirements
- Active LPN license in good standing
- Long-term care or skilled nursing experience preferred
- Strong assessment, documentation, and organizational skills
- Ability to manage multiple admissions and priorities
- Professional communication and attention to detail
- Reliable attendance for a Monday–Friday schedule
Responsibilities
- Complete initial nursing assessments for new admissions
- Review hospital and referral documentation for accuracy and completeness
- Coordinate admissions with nursing, therapy, social services, and administration
- Verify physician orders and initiate care plans upon admission
- Ensure admission documentation meets regulatory and facility requirements
- Communicate with families, hospitals, and referral sources as needed
- Assist with resident room readiness and admission flow
- Dealing with Referrals
- Uploading in PCC
- Tours
- Rotating on call with our Marketer
- Perform other duties as assigned
