About The Position

Requirements

• Licensed as a Registered Nurse in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
• Prior Home Health Nursing experience.
• Proficiency with Medicare & OASIS documentation.
• Demonstrated ability to work autonomously.
• Excellent at planning, organizing, and communicating.

Responsibilities

• Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro.
• Evaluate and assess clients for home health services and create their plan of care.
• Recommend services from other disciplines that have not yet been ordered by the physician.
• Collaborate with clients, families, physicians, and other members of the home care team.
