About The Position

The Admissions Nurse in a home setting provides and arranges for hospice care for patients in a collaborative relationship with members of the Interdisciplinary team. The Admissions Nurse makes initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family. In addition, The Admissions Nurse gathers important contact information, reviews and documents the patient’s medication list and takes their full medical history.

Requirements

  • Must have one year of staff nurse experience in a comparable setting/unit.
  • Hospice and/or palliative care experience is strongly preferred.
  • Must have a degree in Nursing from an accredited nursing program.
  • A BSN degree is preferred.
  • Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.
  • Licensure from a compact state is also accepted.
  • Healthcare Provider BLS (CPR) certification is required to practice in the State of Maryland and the District of Columbia.
  • A CHPN certification is a plus.
  • Must have a driver's license in good standing and access to a vehicle during working hours.

Responsibilities

  • Provide and arrange for hospice care for patients and families in accordance with public health nursing standards, practices, and policies.
  • Supervise LPNs, CNAs, Unit Secretaries, and Volunteers.
  • Make initial nursing assessments and treatment decisions based on patient and family needs.
  • Gather important contact information and review/document patient medication lists and medical histories.
  • May be asked to rotate to other regions or departments.
