The Admissions Nurse plays a key role in hospice care as the first point of contact for patients and families facing a terminal diagnosis. In this position, you will travel to patients wherever they call home, complete comprehensive assessments, and gather the clinical details needed to ensure timely, appropriate care. You will also provide compassionate education and support as families begin their hospice journey. Admissions Nurses help develop individualized care plans, identify immediate and ongoing needs, and guide families with clarity, empathy, and professionalism. The trust and comfort you build during these initial moments create a strong foundation for the entire hospice experience, helping families move forward with dignity, understanding, and peace.
