About The Position

The Admissions Registered Nurse (RN) is responsible for managing the referral and intake process for new hospice patients. This role completes comprehensive admission assessments, verifies eligibility, and provides clear education to patients and families about hospice services and benefits. The Admissions RN ensures a smooth, compassionate transition into care and assists clinical teams when admission or intake volume is low.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current RN license in the State of Ohio.
  • Minimum 2 years of nursing experience in Medical/Surgical, Oncology, or Community Health.
  • Valid driver's license and ability to travel between patient locations (including in inclement weather).
  • Strong communication, customer service, and organizational skills.
  • Detail-oriented with excellent documentation and time-management abilities.
  • Ability to work independently and follow clinical and administrative instructions.
  • Ability to lift up to 25 lbs of equipment or supplies.
  • Physically able to assist with patient mobility tasks such as ambulation, transfers, and positioning.
  • Requires periods of sitting, standing, walking, bending, lifting, and turning.

Nice To Haves

  • Hospice or palliative care experience preferred.

Responsibilities

  • Conduct thorough and accurate initial nursing assessments for new hospice patients.
  • Educate patients and families about hospice services, Medicare/Medicaid benefits, and private insurance coverage.
  • Determine when palliative care interventions or ABNs are appropriate.
  • Obtain, process, and enter physician and Medical Director orders.
  • Coordinate and order appropriate Durable Medical Equipment (DME).
  • Provide skilled nursing care including symptom management, patient/family education, and emotional support.
  • Support clinical operations when admissions are low, including: Care Center assistance End-of-life/continuous care Routine home care visits Chart reviews
  • Ensure all documentation meets agency, accreditation, and regulatory standards.
  • Work evenings and weekends as needed to support patient and family needs.
  • Perform other duties as assigned.
