About The Position

The Admissions Registered Nurse (RN) at Bristol Hospice plays a crucial role in educating the community, providers, and patients about hospice care. This position involves conducting hospice consultations and evaluations, following up on patient referrals, and collaborating closely with the Medical Director or Attending Physician to determine hospice eligibility criteria. The role is designed for those looking to step away from bedside care while still meeting the needs of patients and families, ensuring a compassionate and respectful approach to hospice services.

Requirements

  • Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor's degree in nursing from an accredited program.
  • Must have a current RN license in the state.
  • Must have excellent observation skills.
  • Must possess exceptional verbal and written communication skills.
  • Must possess excellent problem-solving skills and basic math skills.
  • Must be able to travel 20% of the time, often without notice.
  • Must be flexible in work hours.
  • Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.

Responsibilities

  • Educate the community, providers, and patients on hospice care.
  • Complete hospice consultations and evaluations.
  • Follow up on patient referrals.
  • Work closely with the Medical Director or Attending Physician regarding hospice eligibility criteria.
  • Facilitate communication with agency and referral sources to promote the continuum of care.
  • Maintain comprehensive knowledge of hospice educational resources and share information with appropriate personnel.
  • Coordinate and communicate with hospice agency leadership and referral sources to promote care coordination and timely admissions.
  • Complete initial assessments of patients and families to determine hospice needs.
  • Provide patient/family education related to the patient/family plan of care.
  • Participate in the Quality Assurance Performance Improvement (QAPI) process as directed.
