About The Position

Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks an Admissions Nurse Liaison to join its expanding Intake and Admissions team. Travel is required for this position. Care is provided in patients’ private homes and residential facilities throughout Greater Boston and parts of Metrowest. The Admissions Nurse Liaison (ANL) is responsible for ensuring a comprehensive and smooth admission process for patients being admitted to Good Shepherd Community Hospice. The ANL provides explanation of hospice services to patients, families and referral sources, conducts clinical assessments, coordinates admissions, and maintains professional relationships with referral sources and Good Shepherd clinical staff. Requirements

  • Licensed by the Commonwealth of MA as a Registered Nurse
  • At least two years of experience preferred in community health, oncology, hospice, or palliative care
  • Highly skilled at providing patient assessments and evaluations
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families and referral sources
  • Excellent problem-solving skills
  • Competency with accepted standards of hospice nursing practice
