About The Position

The position involves assisting in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to care. The role requires developing a consultative relationship with assigned accounts through effective customer contacts and presentations. Responsibilities include evaluating patients for potential hospice appropriateness by reviewing medical records as ordered by the physician, obtaining agreement for the admission of referred patients and families who meet admission criteria, and ensuring they make informed decisions about the services. The position emphasizes maintaining clinical and professional competence, particularly in clinical appropriateness, pain and symptom management, eligibility requirements, and care coordination. Additionally, the role involves taking the initiative to develop effective relationships with senior management, admissions, and patient care colleagues, along with other duties as assigned.

Requirements

  • Previous healthcare sales or service experience preferred.
  • Previous hospital, physician practice, LTC or ALF experience preferred.
  • Strong presentation skills.
  • Excellent interpersonal skills.
  • Knowledge of clinical manifestations of diseases.
  • Ability to learn VITAS pain and symptom management guidelines.
  • Reliable means of transportation.

Nice To Haves

  • Bachelor's degree preferred.
