With the important role of Admissions Hospice RN, you will provide emotional support and care to patients, helping them to live their final days with dignity and comfort.

Requirements

• Current Arizona RN License in good standing
• Minimum of 1 year of hospice experience
• A warm, empathetic approach to patient care
• Willingness to participate in field visits and team meetings
• Adaptability to changing patient needs

Responsibilities

• Conducting assessments of patients’ physical and emotional needs
• Must be willing to drive to patient homes
• Implementing and updating personalized care plans
• Providing guidance to patients and their families on end-of-life care and disease processes
• Supervising care provided by team members
• Documenting clinical notes and participating in interdisciplinary meetings
