Clinician-led teams built on compassion and respect A supportive environment where every voice matters Commitment to dignity for patients, families, and staff Integrity and empathy at the core of all we do The Admissions Coordinator plays a critical role in facilitating timely and accurate hospice admissions by coordinating referrals, patient records, physician orders, and patient/family appointments. This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team. This will be a hybrid role

Requirements

  • Active, unrestricted Registered Nurse License
  • Be available at all times during agency operating hours or as needed
  • The ability to make sound professional clinical judgment
  • Excellent verbal and written communication skills
  • Proficiency with EMR and data entry systems

Nice To Haves

  • Hospice experience preferred

Responsibilities

  • Receive and process referrals from Community Liaisons, physicians, patients, families, and community partners.
  • Coordinate requests for patient records.
  • Coordinate and schedule patient and family appointments for informed consent and RN assessment visits.
  • Verify and upload patient records to EMR.
  • Ensure timely notification of new referrals, admissions, and non-admissions to the assigned interdisciplinary team (IDT) and ancillary providers.
  • Coordinate supply and DME provision to meet patient needs.
  • Follow up on patients who have not been admitted and/or have been discharged.
  • Maintain and organize patient charts, ensuring all documentation is complete, signed, and dated.
  • Transfer completed admission charts to the Team Coordinator in a timely manner.
  • Explain hospice services, benefits, and eligibility to patients, family members, and referral partners, as needed.
  • Serve as a compassionate and knowledgeable point of contact during the admission process.
  • Inform appropriate team members of scheduled admission appointments and updates.
  • Communicate identified needs, issues, and potential solutions to the supervisor.
  • Attend and participate in staff meetings, educational programs, and community events, as requested.
