The Admissions Coordinator plays a vital role in managing the admission process for patients entering the skilled nursing facility. This position involves coordinating referrals, verifying insurance, and ensuring a seamless transition for new residents.

  • Review clinical documentation to determine level of care needed.
  • Verifies physician orders.
  • Obtain pre-admission documentation from referral sources to determine if agency can care for the patient adequately.
  • Communicate with patients, families, and healthcare providers to gather necessary documentation.
  • Maintain accurate records in electronic medical systems.
  • Coordinate scheduling and follow-up for new admissions.
  • Previous experience in healthcare admissions, intake, or patient services preferred.
  • Strong organizational and communication skills.
  • Ability to multitask in a fast-paced environment.
  • Proficiency with EMR systems and Microsoft Office Suite.
