About The Position

An organized and compassionate Admissions Coordinator with over a year of experience in healthcare and customer service. Demonstrates excellent customer service skills, both in-person and over the phone, and possesses a deep understanding of Skilled Nursing, Homecare, and Hospice criteria. Proficient in multitasking in a fast-paced environment, with strong organizational and analytical skills. Thrives in both independent and team-based work settings.

Requirements

  • High school graduate or equivalent.
  • Demonstrate excellent customer service skills, both in-person and over the phone.
  • Ability to work independently and as a part of a team.
  • Basic computer knowledge.
  • Demonstrate excellent organizational and analytical skills.
  • Ability to multitask in a fast-paced environment.
  • Good verbal and written communication skills.
  • Minimum of 1 year experience in healthcare.
  • Knowledge of Skilled Nursing, Homecare, and Hospice criteria.
  • Minimum of 1 year experience in customer service or call center.

Nice To Haves

  • CNA/MA preferred, but not required.

Responsibilities

  • Coordinate admissions for homecare, hospice, and skilled nursing facilities.
  • Obtain pertinent pre-admission documentation from referral sources to ensure the agency can adequately care for the patient.
  • Review clinical documentation to determine the level of care needed for patients.
  • Verify physician orders for accuracy and completeness.
  • Develop and maintain relationships internally within the organization and with external community partners.
  • Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families to ensure a smooth admissions process.
  • Provide excellent customer service to patients, families, and healthcare professionals both in-person and over the phone.
  • Maintain accurate records and documentation related to patient admissions and care.
