About The Position

Chatham Glen Healthcare and Rehabilitation Center is seeking an organized, compassionate, and detail-oriented Admissions Coordinator to join our team! This role is essential in welcoming new residents, supporting families through the admissions process, and ensuring a smooth transition into our facility. We are hiring for Full-Time positions. ✨ What You’ll Do Provide a warm, welcoming experience to potential residents and families during tours, calls, and inquiries. Coordinate and manage the full admissions process—including assessments, payer verification, hospital day checks, referral tracking, and room assignments. Work closely with Nursing, Social Services, Business Office, and Marketing to ensure timely and accurate admission information. Complete admission packets, obtain necessary signatures, and maintain proper documentation within required timeframes. Verify Medicare/Medicaid, Managed Care, and insurance benefits, including co-pays and prior facility stays. Ensure accurate census reporting, face sheet updates, and payer source documentation. Serve as a liaison between staff, residents, families, and referral sources. Assist with hospital visits when needed and support resident adjustment post-admission. Participate in daily department meetings and prepare monthly admissions/trends reports.

Requirements

  • Experience in healthcare admissions, case management, or similar roles.
  • Strong customer service, interpersonal, and communication skills.
  • Ability to multitask, work independently, and maintain accuracy in a fast-paced environment.
  • Proficiency with Microsoft Word, Excel, Outlook, and related software.
  • Ability to successfully pass a background screening through the AHCA Clearinghouse (For more information on the Clearinghouse, please visit: info.flclearinghouse.com)

Nice To Haves

  • Knowledge of Medicare, Medicaid, Managed Care, and insurance processes (preferred).
