About The Position

The Admissions Support Specialist will support critical document tracking and retrieval to assist the account executives in expanding business opportunities to Hospitals and other community partners. Essential Job Responsibilities Responsible for generating Medicare referrals for hospice and/or home care by building relationships with social workers, case managers and key decision makers. Responsible for monitoring intake platforms such as Navi, All Scripts. Responsible for assisting sales and intake teams with review of incoming documentation to ensure compliant start of care. Will liaison for the organization with referral sources, patients, families as needed/directed to support the admission process and ensure receipt of necessary documentation. Responsible for requested reporting from hospital platforms when requested, as well as participation in sales/marketing meetings. May also assist with other duties as assigned. Must be able to demonstrate team behavior and must be willing to promote a team –oriented environment. Is expected to always represent the organization professionally. May be required to assist weekends/holidays as requested to ensure department is covered appropriately. This may require you to report to office as needed.

  • An ability to read, write and type.
