Job Summary We are seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to complete comprehensive initial assessments for patients and families entering hospice care. The Admissions RN evaluates hospice eligibility, determines patient and family needs, and coordinates care using available community resources. This role focuses on home-based hospice and palliative care services, ensuring comfort, dignity, and quality of life for patients in their homes and communities. Requirements

  • Active California Registered Nurse (RN) license
  • Minimum 2 years of RN experience.
  • Strong ability to work independently, organize workload, and prioritize patient needs
  • Current CPR certification
  • Valid driver’s license with reliable, insured vehicle (state and organizational requirements)

Nice To Haves

  • Hospice, home health, or public health nursing experience preferred

Responsibilities

  • Perform comprehensive physical assessments and obtain complete medical histories of current and prior conditions
  • Conduct hospice admission evaluations and determine appropriateness of hospice services
  • Utilize the full nursing process to provide professional hospice and palliative nursing care
  • Develop, initiate, and document individualized plans of care based on assessment and nursing diagnoses
  • Collaborate with patients and families to establish comfort-focused goals of care
  • Administer medications and treatments as ordered by the physician
  • Educate patients and caregivers on disease progression, medications, diet, and self-care techniques
  • Counsel patients and families regarding emotional, physical, and end-of-life care needs
  • Assist patients with activities of daily living (ADLs) and promote comfort and self-sufficiency when possible
  • Accurately complete and submit clinical documentation, including changes in condition and care outcomes
  • Communicate patient status and care needs with physicians and interdisciplinary care team members
  • Recognize and utilize opportunities for patient and family education and health counseling
