About The Position

The Admission Nurse is responsible for assuring that SAH’s service lines are effectively presented to patients, families, and others who have an interest in hospice or palliative care. They direct the conversion of referrals to admissions for general inpatient hospice, home hospice, and palliative care throughout the St. Anthony's Hospice service area. Upon determining that disease-specific eligibility requirements are met, the Admission Nurse completes an accurate patient admission assessment of initial physical and psychosocial needs, including supplies, durable medical equipment and medications. When not committed to a scheduled referral or admission visit, the Admission Nurse works in collaboration with the Director of Marketing & Development and allocates time each working day for outreach efforts to hospital case managers / discharge planners and other identified professional referral sources. The Admission Nurse must exercise independent judgment to manage their weekly schedule. Understanding that the workload may vary from day to day, adjustments in the Admission Nurse's scheduled work hours may need to be altered in the interest of patient care.

Requirements

  • Graduate of an accredited School of Nursing required.
  • Licensed as a Registered Nurse or Licensed Practical Nurse by the Commonwealth of Kentucky
  • A minimum of one year nursing experience required, two years preferred (preference given to applicants who have hospice experience, have worked with terminally ill patients or have experience in pain management)
  • Demonstrated ability to work independently, establish priorities and meet agency goals
  • Strong organizational and interpersonal communication skills – both written and verbal
  • Must possess the personal qualities of responsibility, dependability, empathy, ability to positively influence others and problem solve
  • Must meet all legal and regulatory requirements of both federal and state governments as well as Medicare Hospice Conditions of Participation.

  • Proven experience in the areas of marketing or community outreach preferred, but not required

Responsibilities

  • Establishes positive relationships with patients, caregivers, referral sources, fellow staff members and other healthcare professionals.
  • Effectively communicates accurate information regarding hospice and palliative care to patients, caregivers and referral sources.
  • Articulates and demonstrates a professional image daily to patients, peers and the public.
  • Utilizes time not committed to scheduled intake visits to contact potential referral sources.
  • Collaborates with Referral Specialist in a timely manner.
  • Collaborates with the Director of Nursing when necessary.
  • Documents all contacts, phone calls and assessments made during the referral and admission processes on the appropriate forms / communication tools at point of care.
  • Other duties as deemed necessary or needed.
  • Conducts intake visit with patient and/or caregivers, gathers needed information to determine eligibility of patient, and provides needed information to facilitate decisions regarding appropriate level of care for each patient.
