About The Position

The position is for a part-time Hospice Admission Nurse servicing communities around Advocate Masonic Hospital, Advocate Trinity, and Advocate Christ Hospital. The role requires a commitment of 20 hours per week and includes weekend and holiday rotations. The nurse will be responsible for leading the interdisciplinary care team for each patient and family on their caseload, ensuring appropriate admissions to hospice services, and coordinating care with various healthcare professionals.

Requirements

  • Registered Nurse license issued by the state in which the team member practices.
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
  • A valid driver's license issued by the Division of Motor Vehicles.
  • Associate's Degree in Nursing.
  • Typically requires 2 years of direct patient care experience.

Responsibilities

  • Responsible for the admission to hospice services and leading the interdisciplinary care team specific to each patient and family.
  • Obtains history and completes physical assessment to evaluate appropriateness for hospice services.
  • Meets with patients and families to explain the hospice program and obtain informed consent.
  • Reviews insurance/Medicare eligibility and documents data to support billing efforts.
  • Assesses patient and family readiness and involves them in planning and implementing care.
  • Coordinates all services and collaborates with the interdisciplinary team to provide consistent patient care.
  • Facilitates appropriate placement in hospice contracted facility if necessary.
  • Assesses need for and coordinates delivery of medications, supplies, and home medical equipment.
  • Demonstrates effective communication and conflict resolution skills.
  • Develops and provides hospice education to inpatient hospital and facility staff.
  • Acts as an expert resource to hospital and facility staff regarding hospice and palliative care services.
