Addictions Nurse Care Manager
About The Position
The Addictions Nurse Care Manager navigates and coordinates the care of patients across the continuum of care with a focus in Addiction’s Medicine. Responsible for coordinating admissions and referrals for inpatient and outpatient services, verifying insurance coverage, collecting patient information, and ensuring compliance with regulatory and organizational standards. Assesses, plans, coordinates, monitors and evaluates the patients’ plan of care in collaboration with the healthcare team. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies.
Requirements
- Associate’s in Applied Science in Nursing
- Minimum of two (2) years of experience in acute/tertiary facility
- NYS Registered Nurse

- Bachelor’s of Science in Nursing
- Five (5) years of experience in acute/tertiary facility
