Requirements

• Associates degree in Behavioral Sciences.
• Licensed Alcohol and Drug Counselor - MN Board of Behavioral Health & Therapy required upon hire.
• Must meet at least ONE of the requirements: Current BLS certification from the American Heart Association, Current BLS certification from the American Red Cross, or Allina in-house BLS training (within 30 days of hire).

Nice To Haves

• Bachelor's degree in Behavioral Sciences.
• Experience in a treatment program.

Responsibilities

• Perform initial assessment and screen for admission to addiction services.
• Complete comprehensive biopsychosocial assessment of patient.
• Reassess patient and progress in treatment.
• Provide crisis support and management.
• Meet requirements for 245G and third-party payers.
• Complete treatment plan per department process.
• Document patient and family participation in development of treatment plan.
• Review treatment plan with patient at regular intervals.
• Initiate referrals to internal/external providers and agencies.
• Coordinate discharge and transition planning.
• Facilitate group and individual counseling as identified by treatment plan.
• Coordinate treatment with other community agencies/providers.
• Provide patient and family education as needed.
• Perform other duties as assigned.
