The Addiction Counselor will be responsible for providing individual and group level Substance Use Disorder/Co-Occurring Disorders (SUD/COD) interventions, Instituting Trauma Informed Care, Motivational Interviewing, and "Seeking Safety" in key areas, and managing substance abuse treatment modalities via the SAMHSA - funded IMPACT program. Implement comprehensive behavioral health and substance use assessments and assist clients in developing goals and services/treatment plans that address identified SUD/COD needs. Identify the treatment services based on severity of substance use diagnosis, stage of change and immediate needs and recommend treatment objectives for the individual client's service plan. During the assessment obtain information to establish the degree risk for substance use and associated harmful behaviors, and interventions required to promote progress towards recovery. Address addictive behaviors with clients on a regular basis through individual and group sessions to promote relapse prevention. Lead the care team in discussing substance abuse issues in weekly case conference. Develop comprehensive service plans and six-month service plan review in collaboration with the client and the client's support system. Monitor clients’ behavior and compliance with care plan. Involve family and significant others in the client's treatment with the care team and collaborate effectively with the client's support system and community partners involved in the client's well-being. Complete referrals and linkage to community partners who provide additional treatment services such as help recovery groups, intensive treatment options like detox and rehabilitation. Complete crisis intervention and relapse prevention plans, progress notes and significant data. Provide individual mental health, relapse prevention, harm reduction and substance abuse counseling based on motivational interviewing and Trauma-Informed Care. Assess and provide support to clients in addressing needs for problem-solving, wellness self-management, housing, incoming support, education and vocational training, social supports, employment, and primary care. Function as advocate and liaison for clients in securing emergency detox and rehabilitation care, and other community supports that promote integrated dual disorder treatment. Maintain accurate, up-to-date, and complete client charts, utilizing program-specific documentation templates, and enter client data in a timely fashion in program-related databases. Attend necessary trainings and in-service to remain abreast of best practices in addiction treatment and care. Utilize with fidelity program-specific evidence-based interventions and other quality service delivery strategies to implement appropriate client-centered counseling and education. Participate in regular supervision with Clinical Supervisor and Project Manager and in staff and team meetings as required. Facilitate supervision with Program Peer Educators. Complete in timely fashion other job-related duties to meet program needs as assigned by the Program Manager and the Director of Supportive Services. Demonstrate effective communication skills necessary to interpret age-appropriate response to client service/treatment plan. Perform other duties as assigned.
