About The Position

The Addiction Counselor (CASAC) at Wyckoff Heights Medical Center will provide individual and group interventions for clients with Substance Use Disorders and Co-Occurring Disorders as part of the SAMHSA Project IMPACT. The role focuses on implementing trauma-informed care, motivational interviewing, and comprehensive treatment planning to support clients in their recovery journey.

Requirements

  • Bachelor's degree in a relevant field.
  • CASAC Certification with current New York State License.
  • 3-5 years of experience working in human services, particularly with individuals with mental illness and substance use disorders.
  • Experience with motivational interviewing and evidence-based treatment models.

Nice To Haves

  • Master of Social Work (MSW) degree.
  • Proficiency in Spanish.
  • Experience working with co-occurring disorders.

Responsibilities

  • Implement comprehensive behavioral health and substance use assessments.
  • Assist clients in developing goals and treatment plans addressing identified SUD/COD needs.
  • Identify treatment services based on severity of substance use diagnosis and recommend treatment objectives.
  • Obtain information during assessments to establish risk for substance use and harmful behaviors.
  • Provide relapse prevention information through individual and small group sessions.
  • Lead care team discussions on substance abuse issues in weekly case conferences.
  • Develop comprehensive service plans and conduct six-month service plan reviews with clients and their support systems.
  • Monitor clients' behavior and compliance with care plans.
  • Involve family and significant others in treatment and collaborate with community partners.
  • Complete referrals and link clients to community partners for additional treatment services.
  • Conduct crisis intervention and develop relapse prevention plans and progress notes.
  • Provide individual mental health and substance abuse counseling based on motivational interviewing and trauma-informed care.
  • Assess and support clients in addressing needs for problem-solving, wellness self-management, and vocational training.
  • Advocate for clients in securing emergency detox and rehabilitation care.
  • Maintain accurate and complete client charts and enter data in program-related databases.
  • Attend trainings to stay updated on best practices in addiction treatment.
  • Utilize evidence-based interventions and quality service delivery strategies.
  • Participate in regular supervision and team meetings.
  • Facilitate supervision with Program Peer Educators.
  • Complete other job-related duties as assigned.
