M Health Fairview has a 0.9FTE Bone Marrow Transplant Social Worker opening! The hours are 8am-4:30pm with every other Friday off. This position works both oupatient and inpatient at M Health Fairview- The University of Minnesota Medical Center in Minneapolis following BMT patients across the continuum of care. They are responsible to do new transplant evaluations, psycho-social assessments, follow patients during their transplant hospitalization and support patient post-transplant. There is also an every 6th weekend component working inpaitent at The University of Minnesota Medical Center Hospital and 1 Holiday per year requirement. The information below pertains to the inpatient role/ portion of this position. The acute social worker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute social worker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The social worker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
