About The Position

The Acute/SNF Physical Therapist II position is a part-time role located in Chula Vista, CA. The therapist will evaluate, plan, direct, and administer physical therapy modalities of treatment as prescribed by a licensed physician. The role involves assisting patients in reaching their maximum performance and level of functioning while learning to live within the limits of their capabilities. Requirements

  • Graduate of an approved and accredited professional Physical Therapy school or program.
  • 2 years of experience in all aspects of physical therapy.
  • California Licensed Physical Therapist (PT) - CA Physical Therapy Board - REQUIRED.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED.

Responsibilities

  • Evaluates, plans, directs and administers physical therapy modalities of treatment as prescribed by a licensed physician.
  • Assists patients in reaching their maximum performance and level of functioning.
  • Participates in operational aspects of the department and maintains performance improvement activities.
  • Participates in CQI activities.
  • Acts as a clinical resource and is consulted as a representative of the profession.
  • Identifies clinical issues requiring education and participates in the implementation of educational plans.
  • Provides in-service/education programs or training.
  • Participates in departmental activities such as CQI, staff meetings, and chart review.
  • Completes documentation in a timely, complete, legible, concise, and accurate manner.
  • Communicates patient care and departmental issues effectively.
  • Designs, implements, and evaluates treatment effects, recommending changes to physicians as needed.
  • Assesses patient pain and provides appropriate interventions.
  • Formulates teaching plans based on identified learning needs.
