As an Acute Medication History Pharmacy Technician, you will be responsible for speaking with patients who are going to be admitted to the hospital about the medications they are taking at home. It is expected that you are able to look at medication bottles and correctly interpret the directions, correctly identify unlabeled tablets or capsules, discuss any inconsistencies with the patient and/or research issues with a pharmacist, the patient's family or outside pharmacies. You will build an accurate medication list and enter appropriate notes in Cerner with a goal of developing an accurate medication list in Cerner for use by the nurses and physicians during a patient's stay and to help ensure a safe discharge to home. This position, under the supervision of a pharmacist, is responsible for obtaining a patient's complete medication history through communication with the patient, family, and/or other resources and documenting it into the medical record. Medication History technicians (MHTs) perform an important function in collecting an accurate and complete medication history from every patient they encounter. A complete medication history will include prescription medications, over-the-counter (OTC) products, herbal supplements, vitamins/minerals, and other similar products.
