Northwestern Counseling & Support Services, Inc. is seeking a dedicated Acute Care Consultant to join our dynamic Stabilization Support Team. This role offers the opportunity to lead with compassion, coordinate care across systems, and create meaningful impact in the lives of our most clinically acute clients. As an Acute Care Consultant, you'll work in close partnership with the Children’s Home and Community Based Program Manager to support team operations, guide clinical processes, and ensure timely access to higher levels of care for youth in the Franklin Grand Isle region. You’ll serve as a key liaison between NCSS and community partners such as DCF, DMH, and local providers—helping to break down barriers and build bridges that lead to lasting stability for the children and families we serve. The ideal candidate will bring good collaboration experience, strong organizational skills, and a family-centered approach to care coordination. You’ll offer consultation and support to team members, contribute to quality improvement efforts, and occasionally provide direct services for clients receiving respite, psychiatric care, or waiting for placement. This position offers a meaningful blend of systems-level coordination, team leadership, and direct clinical impact, all within a supportive and mission-driven environment.
