ACTT is a comprehensive team of professionals from many disciplines that provide tailored services to meet the client’s mental health, substance use, vocational and housing needs. ACTT improves outcomes for people with severe mental illness who are most at risk of psychiatric crisis and/or affected by other social determinants of health.

Requirements

  • LCMHC(A), LCSW(A), LMFT and clinical experience working with mental illness and substance abuse
  • Required Licensure: LCSW, LCMHC, LMFT, Licensed Psychological Associate, APN, Licensed Psychologist, Licensed Psychiatric Nurse Practitioner, Clinical Nurse Specialist certified as Advanced Practice Psychiatric Clinical Nurse Specialist
  • Master's Degree in Human Service or related field from an accredited college or university is required.
  • Required: (3) years clinical experience working with severe and persistent mental health illness, (2) of those years being post-graduate school, and experience with supervision and management of Substance Abuse and Mental Health services

  • (5) years of experience working with severe and persistent mental health

Responsibilities

  • Provide clinical leadership and oversight in collaboration with the psychiatric care provider(s), to the ACT program
  • Supervise and manage the team operations and staffing.
  • Ensures the systematic provision of treatment and social services such as case management, individual, group and family therapy.
  • Responsible for communication among staff, consumers, families, and external resources for collaboration and continuity of treatment
  • Actively participates as a part of a multi-disciplinary treatment team.
  • Responsible for assessment, treatment and discharge planning.
  • Team Lead maintains positive relationship with referral sources, stakeholders, payors and TMACT.
  • Other duties as assigned
