Requirements

High School diploma or equivalent
Experience working with students with complex medical needs and intellectual disabilities.
Ability to pass criminal background check and drug testing
Must be able to lift, push, pull or carry a minimum of 5 0 lbs.

Nice To Haves

Experience working with various socioeconomic populations is a plus.

Responsibilities

Implements activity and after school programs according to the written schedule
Document progress and attendance for all activities (both group and individual)
Completes documentation daily
Assists with completion and implementation of documentation of habilitation plans
Maintains records and prepares requested reports
Complete activity notes by the 10 th of each month
Participates in the continuing interdisciplinary evaluation of individual residents needs
Assures there is written permission given by the resident's physician to participate in the activity program
Implements the activities to coordinate with other departments to e nsure communication between all departments to provide uniformity of goals
Participates in transporting students to and from the activities
Assist in the development and implementation of special events
Assist in the coordination of holiday visits from various groups/foundations
Suggest ideas for in-house and community activities
Attends seminars/workshops/ facility/department meetings as necessary
