About The Position

The Social Services Director/Activities Director at Auburn Skilled Nursing and Rehabilitation is responsible for developing and managing a comprehensive social services and activities program that aligns with the facility's vision and values. This role involves conducting evaluations, refining care initiatives, and ensuring that the needs of residents are met through individualized care planning.

Requirements

• Broad understanding of social services and activity programming in a skilled nursing setting.
• Excellent communication skills.
• Commitment to resident care.

Nice To Haves

• 1 year of experience in activities (Preferred)
• 1 year of experience in social services (Preferred)

Responsibilities

• Prepare and maintain reports summarizing the preferences and psychosocial history of residents.
• Provide leadership and direction in the development of the activity program, including planning and implementing the daily calendar.
• Review and monitor activity staff to ensure their goals, objectives, and behavior are consistent with facility policy.
• Ensure that all required social service assessments are completed and that the needs of residents are individualized and care planned.
• Oversee the discharge planning process for residents.
• Ensure preadmission screening tools are completed as required by regulation.
• Oversee and implement ancillary care provided to residents, such as podiatry, dentistry, audiology, psych services, vision, and hospice care.
