About The Position

Diablo Valley Post-Acute is looking for a creative, energetic, and compassionate Activities Assistant to join our life enrichment team. We believe that an engaging activities program is essential to our residents' health and happiness. As an Activities Assistant, you will work hand-in-hand with the Activity Director to bring our community to life:

Requirements

• High school diploma or equivalent.
• Preferably at least one year of experience in a long-term care or post-acute facility.
• A positive "can-do" attitude, excellent communication skills, and a heart for service.
• Ability to maintain a clean and orderly Activity Department while managing attendance records and documentation.

Responsibilities

• Plan and conduct individual, small-group, and large-group activities ranging from games and crafts to music and celebrations.
• Help develop the monthly activity calendar to ensure a diverse and exciting schedule of events.
• Facilitate communication between families, staff, and residents to ensure every individual’s interests and needs are being met.
• Develop specialized in-room activities for residents who are bed-bound or isolated, ensuring no one is left out.
• Encourage residents to pursue self-initiated hobbies and provide the necessary materials, such as audiobooks or craft supplies.
• Assist with resident assessments, discharge planning, and documenting quarterly progress notes to track the positive impact of our programs.
• Arrange transportation for residents for exciting outings and community appointments.
