About The Position

The position involves planning and conducting individual, small, and large group activities for residents in a long-term care facility. Responsibilities include participating in community planning, developing a monthly activity calendar, maintaining attendance records, and assisting in quality assurance efforts. The position also involves discharge planning, developing activity care plans, and arranging transportation for residents. The role encourages residents to engage in self-initiated activities and requires the provision of necessary materials, including reading materials in Braille or audio formats. The Activity Department must be kept clean, orderly, and secured.

Requirements

  • High school diploma or equivalent.
  • Preferably one year of experience in a long-term care facility.
  • Ability to read technical procedures and comprehend policy manuals.
  • Ability to effectively present information and respond to questions.
  • Ability to apply mathematical concepts to practical situations.
  • Ability to solve practical problems and interpret various instructions.

Responsibilities

  • Participate in planning and conducting individual, small, and large group activities.

  • Assist in the development of a monthly activity calendar and maintain attendance records.
  • Assist the Quality Assurance and Assessment Committee in developing and implementing plans of action.
  • Participate in discharge planning and development of activity care plans.
  • Assist in arranging transportation for residents for outings and appointments.
  • Encourage residents to participate in self-initiated activities.
  • Provide necessary materials for activities, including Braille or audio books.
  • Assist with assessment documentation and quarterly progress notes.
  • Keep the Activity Department clean, orderly, and secured.
