Who We Need As a member of the Assertive Community Treatment (ACT) Team, the Substance Abuse Specialist works collaboratively with a multi-disciplinary care team to identify, assess, and provide stage-wise engagement, persuasion, active treatment, and relapse prevention/management services in individual and group formats for persons with alcohol, tobacco, and other substance use involvement. Works as a collaborating ACT team member to facilitate recovery and inspire hope for service members while supporting their achievement of service plan goals. to assess service member needs, participate in the development of individual service plans, and provide advocacy and supports for service members, their families and involved community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage on an established schedule.
