Terros Health is hiring an Arizona Licensed Registered Nurse to join one of our ACT Teams in Phoenix, AZ. Position is located near I-17/ Peoria Desert Cove Clinic. Schedule: 4 - 10's.

Requirements

  • EDUCATION: ADN, BSN or MA in nursing.
  • Registered nurse with the Arizona State Board of Nursing.
  • One (1) year prior experience in psychiatric setting and/or working with members receiving behavioral health services.
  • Experience working within psychiatric crisis and/or with members with serious mental illness preferred.
  • Interpersonal skills for professional relationships with co-workers, members, families, and community members.
  • Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions.
  • Strong understanding of substance use, addiction, and recovery principles.
  • Knowledge of chronic conditions experienced by those with serious mental illness preferred.
  • Valid Arizona Driver's License and Active AZ Registered Nurse License.
  • Must have a valid Arizona Fingerprint Clearance card or ability to apply for one within 7 working days of assuming role.
  • Must pass a drug screen and TB Test.

Nice To Haves

  • Knowledge of electronic medical records, NextGen preferred.
  • Experience with Microsoft Word, Excel, PowerPoint, and Outlook.

Responsibilities

  • Provides direct clinical services to individuals, groups, and/or family members in community-based settings.
  • Coaches members and families on behavioral and physical health conditions, self-management techniques.
  • Promotes treatment adherence and personal wellness within a recovery framework.
  • Coordinates member care within the clinical team and with external parties including the member's primary care physician.
  • Attends Primary Care and Specialty appointments with the client.
  • Demonstrates critical thinking and takes initiative, working independently within the scope of practice.
  • Performs health evaluations, takes vitals, draws blood for lab tests, assists with medication monitoring, and administers intramuscular medications.
  • Responsible for the oversight and monitoring of medication delivery, observation, and administration for the ACT Team clients.
  • Actively participates as part of the member's clinical team.
  • Takes full responsibility for the integrated treatment services for members.
  • Works with informal support systems to provide clinical skills training.
  • Maintains compliance with ACT fidelity measures.
  • Utilizes assertive engagement mechanisms to re-engage members in prescribed treatment.
  • Routinely assesses member's need for ACT services and transitions eligible members to less intensive services.
  • Responsible for crisis services for assigned members, coordinating hospital admissions and discharge planning.
