Terros Health is hiring a Psychiatric Nurse Practitioner to join our ACT Team at our 23rd Avenue Recovery Health Center location in Phoenix. Full-Time; 4-10's 23rd Avenue Recovery Health Center - I-17/ Dunlap Salary Range: $140K-$160K: MSN: $140K-$155K DOE, DNP: $158K-$160K Additional ACT Differential Pay: $5/hour ($10,400 Annually) Additional Language Differential Pay Available: Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL: $3/hour ($6240 annually) The ACT Nurse Practitioner serves as an active member of the site management team working collaboratively with the Lead Nurse on the delivery of clinical care. The ACT Nurse Practitioner conducts both home and community visits as well as in clinic care.

Requirements

Advanced Practice Nursing degree as a Psychiatric Nurse Practitioner
Must have one (1) or more year’s prior experience in psychiatric setting and/or providing psychiatric treatment; experience working within psychiatric crisis and/or with members with serious mental illness preferred
Must have active license to practice with AZ Nursing Board. Board certified.
DEA and AZ CSPMP registration current
Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy
Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
Must pass a Drug Screen and TB Test.

Responsibilities

Provides time-unlimited, community-based services to assigned caseload size of 10:1, ensures intake rate is low to maintain a stable clinical environment for other members on the case load.
Actively participates as part of the member’s clinical team
Takes full responsibility for the treatment services for members; coordinates care with ACT team members and other providers to ensure member’s needs are met and the member continues to receive treatment in the community to the greatest extent possible
Works with informal support system with or without the member present to provide clinical skills training to increase informal supports (family, landlords, employer etc.)
Maintains high intensity of services and high-frequency contact with members as needed
Supports assertive engagement mechanisms, including street outreach and legal mechanisms (probation/parole, court ordered treatment), when necessary, to re-engage the member in prescribed treatment.
Routinely assesses member’s need for ACT services, using explicit criteria to determine need to transfer to less intensive services.
Gradually and individually transitions eligible members and ensure continuity of care.
Monitors member’s status during transition and reintroduces services as needed to ensure member’s stability in the community.
Is responsible for crisis services for assigned members, coordinating hospital admissions and discharge planning.
Participates in daily team meeting; provides updates on members, identifies member’s risks and needs and works with ACT clinical team to coordinate care to access needed services.
Serves as the subject matter expert in designated role, offering skills training and technical assistance
Shall fully comply with all program requirements per SAMHSA evidence-based practice guidelines, Western Interstate Commission for Higher Education (WICHE), Mercy Maricopa and others as identified by Terros, Inc.
Provides all services to members in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.
Works with other members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
Connects members and families to assistance for accessing additional needed services and obtaining entitlements, including identification of funding services
Assumes “primary contact” responsibility for a percentage of team members to: assure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individuals; keep weekly client schedule for those individuals updates, and act as first contact for those members and their families.
Documents all member events in accordance with policies, procedures, and clinical protocols in a timely manner.
Provides psychopharmacological treatment in alignment with member’s treatment plan
Typically provides at least monthly assessment and treatment of consumers’ symptoms and response to the medications, including side effects
Provides brief therapy
Provides diagnostic and medication education to consumers, with medication decisions based in a shared decision-making paradigm
Monitors all members’ non-psychiatric medical conditions and non-psychiatric medications
If members are hospitalized, communicates directly with consumers’ inpatient psychiatric care provider to ensure continuity of care
Conducts home and community visits as required by ACT team
Collaborates with the team leader in sharing overall clinical responsibility for monitoring consumer treatment and team member service delivery
Educates non-medical staff on psychiatric and non-psychiatric medications, their side effects, and health-related conditions
